Appearance Release Form Sample

An appearance release allows a person appearing on camera to give permission to the producer to use their image, voice and likeness.  It is recommended to get an appearance release for anyone who is recognizable in your footage.  If you shoot someone walking down the street from behind, you don’t need a release.  If someone down the road recognizes themselves on your footage and wants to complain or worse, you will have this document to prove you were allowed to shoot them.  Minors need to have a guardian sign for them. You can manipulate the document to reflect:

• No cost or fee based
• Distribution platforms
• Time allowed to use the person’s image

I like to get rights for all platforms, in perpetuity.

Location Release Form Sample

A location release is a form that allows the owner of a location to give written permission to the producer to shoot at the location.  For example, if you are shooting a piece at a high school, it is a good idea to have a representative of the high school fill out and sign the location release.  If someone down the road recognizes the school on your footage and wants to complain or worse, you will have this document to prove you were allowed to shoot there.  You can manipulate the document to reflect:

This is my favorite tally sheet because it is so quick and easy. It let’s me know how much time is going “INTO” my segment, show or video.  I use it a lot for discussion series where the graphic packages and music are standard on every show.  The “in time” is subtracted from the “out time” to give the duration of the clip. All the clip run times are added up to give the total duration of the show.

Here is the basic format that I use for most longer scripts or for scripts where I need to communicate a lot of graphic/sound direction. In this script I only indicated the “in” time codes for each bite. “Out” Script Cut Sheet To Quickly Tally Duration

This is a sample Excel cut sheet that can be used to tally up how much time you are TAKING OUT OF  the show.  I use this template if I have a long discussion that the editor will be chopping down. I use this instead of scripts  for panel discussion style show that don’t require a lot of graphics. It is also helpful for recutting existing programs.  I use it to get a quick sense of how long a show is going to be. Then, I give graphic and other notes at the bottom of the document or later on in the editing process.

Here is how it works.

1. The “Out Time” gets automatically subtracted from the “In Time” to give the duration of what is being removed.
2. Everything in the duration column gets subtracked from the total lenght the of the master clip.
3. I put the target time just so I remember what is the actual length that I am trying to meet
